PARTICIPATION LIABILITY & WAIVER RELEASE FORM

In consideration of the grant of permission to _______________________________________________ (name of player) by Brandywine Volleyball Club and Wilmington Friends School to tryout and/or practice with the use of the buildings, grounds and/or facilities of the Wilmington Friends School, more particularly the East and West gyms (also the surrounding facilities) for the purposes of volleyball tryouts and/or practices, the undersigned being fully aware of all attendant risks of such activities and being of sufficient age and competency to understand such risks, hereby releases Brandywine Volleyball Club and Wilmington Friends School, it’s agents, administrators, coaching staff and trustees of and from any and all claims for personal injuries, death and property damage or loss which may arise from the use of said building, grounds and /or facilities pursuant to said grant of permission, and do further agree to defend, indemnify and save harmless Brandywine Volleyball Club and Wilmington Friends School its agents, administrators, coaching staff and trustees from any and all claims. I, the undersigned have read and fully understand the terms of this Liability & Waiver Release Form.
